| Students in Kindergarten and First Grade | |
| Children entering kindergarten must be five years old by September 1 and students beginning the first grade must be six years old by September 1. A birth certificate must be presented for verification of the date of birth. Kindergarten and first grade students must have a physical examination from their doctor. Examination forms are available at most doctor’s offices. | |
| Medical Examinations and Immunizations | |
| In addition to the required kindergarten or first grade physical, it is recommended that students entering 7th grade have a medical checkup. All students must conform to state law on immunizations. Students who have incomplete immunization records will be excluded from school after receiving notice from the Lane County Health Department stating non-compliance. | |
